Responsibilities:
- Conduct comprehensive medication reviews and assessments for patients with complex medical conditions, ensuring safe and effective use of medications and optimizing therapeutic outcomes
- Collaborate with multidisciplinary teams, including physicians, nurses, and allied health professionals, to develop and implement individualized care plans and interventions for patients with chronic diseases, acute exacerbations, and polypharmacy issues
- Provide expert advice and support to healthcare professionals on medication management, drug interactions, dosage adjustments, and adverse drug reactions, promoting evidence-based practice and adherence to clinical guidelines
- Participate in ward rounds, patient consultations, and clinical meetings, contributing to discussions on treatment options, medication reconciliation, and discharge planning to facilitate seamless transitions of care for patients across healthcare settings
- Lead and oversee pharmacy services within the clinical area, including medication reconciliation, prescription screening, and medication supply management, ensuring compliance with regulatory standards and best practice guidelines
- Engage in quality improvement initiatives, audit activities, and research projects to evaluate and improve medication-related outcomes, patient safety, and service delivery within the organization
- Provide mentorship, supervision, and training to junior pharmacists, pharmacy technicians, and other healthcare staff, fostering a culture of learning, development, and professional growth
Requirements:
- Degree in Pharmacy from a recognized institution and registration as a Pharmacist with the relevant regulatory body
- Postgraduate qualification in Clinical Pharmacy or equivalent advanced training and experience in clinical practice, with Band 7/8A level expertise and competencies
- Proven clinical pharmacy experience in a hospital or clinical setting, with a strong understanding of pharmacotherapy principles, pharmacokinetics, and pharmacodynamics
- Excellent clinical assessment and decision-making skills, with the ability to prioritize and manage workload effectively in a fast-paced and challenging environment
- Strong communication and interpersonal skills, with the ability to build rapport with patients, healthcare professionals, and external stakeholders and communicate complex pharmaceutical information in a clear and concise manner
- Commitment to professional development and continuous learning, with a proactive approach to staying updated on advances in pharmacy practice, clinical guidelines, and regulatory requirements
- Demonstrated leadership and team collaboration skills, with the ability to work autonomously and as part of a multidisciplinary team to achieve common goals and deliver high-quality patient care
