Exciting opportunity for an experienced Social Worker/Occupational Therapist to take the next step up to more independent working. We are looking for a Social Worker or Occupational Therapist who would be interested in working on a new project with us for at least the next 6 months. The project is for a new Hospital Discharge Pathway 3 Discharge to Assess beds located in a local nursing home (maximum of 16 beds).

The worker would be required to have enhanced assessment skills, knowledge and experience of CHC assessment process and experience with discharge planning. The worker would also need to complete care act assessments and MCA work.
The worker would be working as part of an MDT in the nursing home for people on a short stay 4-6 weeks as a step down from hospital whilst assessing long term needs.
As the role would involve going to a care home, the worker will need to show evidence of being fully vaccinated for Covid-19
Job Summary; To provide a social work service to the client group.  Responsible for the implementation and co-ordination of complex assessments, the purchase of agreed packages of care, and the monitoring of service contracts in partnership with the client group and agencies and health professionals.  Responsible for the provision of casework and other social work services.



Candidate Profile

  • To hold a recognised social work qualification and be able to provide evidence of registration to the GSCC.
  • Knowledge of key legislation relevant to professional social care work
  • Knowledge of the functions of a health and social care department and related agencies
  • Knowledge of methods to plan and review services for the client group
  • Knowledge of social diversity and equal opportunities and the ability to demonstrate this throughout all aspects of practice
  • Must have minimum 4 years Post qualification social work experience required
  • Experience of managing a complex case load
  • Strong influencing and negotiating skills with clients and other health professionals
  • Ability to organise and prioritise work and manage time effectively
  • Able to work independently and within a team.
  • Customer focused
  • Experience of producing reports for a variety of audiences
  • Good IT skills
