What skills are required for the job?
To lead the Access service for the N&E of the county, managing incoming referrals, oversight and support of the triage and prioritising of risk as the cases come into the Access service. Liaison with colleagues from health and social care to ensure the correct pathways and outcomes for referrals.
Working to prevent, reduce delay needs, maximising independence linking with partners in voluntary services such as Gateway/ Humans of the Peninsular.
Undertake Care Act 2014 needs assessments/ and support plans and transfer these cases to PCN teams as appropriate for review.
Any special requirements?
Either able to come into an office or work from home remotely and be a car user to travel in and around North and East Locality Area.
