An exciting opportunity has arisen within the Together Achieving Change (TAC) Team, in Wrexham as a TAC Officer. We are looking for a dynamic and enthusiastic individual, with experience working with children, young people, and families, within social care, health, education, or community settings e.g. family support worker, social worker, youth worker, , teacher, children’s community development worker. You will take the lead in co-ordinating an effective preventative offer to families across Wrexham County Borough Council.
If you apply for this role and are successful, you will be part of an experienced and committed team within the Social Care department. A Degree level qualification in Social Care, Social Work, Education, Health or Youth Work is required for this post.
You will work with partners to provide families with consistent Early Help and Targeted support to improve outcomes in line with the Social Services Well-being Wales Act 2014 and the Wrexham County Borough Council Prevention and Early Help Framework. This will include identifying needs, and providing appropriate preventative support, including the chairing of TAC meetings and reviews.
This is a full – time role, however flexible hours / hybrid working ( home and office ) is available. The role could be up to 6 months for the right candidate.
