  • To hold a recognised social work qualification and be able to provide evidence of registration to the GSCC.

  • Must have minimum 4 years Post qualification social work experience required
  • Experience of managing a complex case load
  • Strong influencing and negotiating skills with clients and other health professionals
  • Ability to organise and prioritise work and manage time effectively
  • Able to work independently and within a team.

  • Experience of producing reports for a variety of audiences
  • Good IT skills
 Social worker required for a specialist post to joint work with Adult Social Care and the local Continuing Health Care team. The candidate should have experience working in a Multi-Disciplinary setting; able to work autonomously, have good understanding of the CHC framework, the Care Act. The Social worker must be able to complete assessments within the target framework. The assessments will be completed virtually and/or face-to-face. The role involves working from home and from the office and the social worker will be on a rota. The applicant will be required to work from Monday to Friday 36 hours a week. This post is funded by the CCG.
