The Lifelong ServicesTransition Teams are designed to focus on working with young people and adults with a disability/autism through their transition to adult social care services. The focus is on starting transition work early in order to ensure this often stressful process can be managed smoothly. The Lifelong ServicesTransition Teams meet the statutory duties of the Council by undertaking assessments of need, support planning, monitoring and reviewing of customers with eligible social care needs. The teams comprise of social workers and care managers who work closely with health colleagues within the integrated lifelong Services Adults Teams. The teams cover the whole of West Sussex and out of area services for people who are the responsibility of West Sussex County Council, and team members will be located with each of the three Lifelong Services Adults Teams in Horsham, Bognor and Worthing. The aim of the teams is to facilitate a smooth transition into adult social care services. To provide an accountable statutory social work service to those people who access or need the service to include assessment, planning, monitoring and review. The post holder is expected to work within organisational, policy and legislative requirements. **Please note - The official locations are Centenary House (Worthing) and Durban House (Bognor). The teams cover coastal, Northern and county wide so potentially both people could work from the Horsham office on occasions. The postholders will work at home, except to collect IT equipment and any direct support or training and direct work with customers. There are no fixed number of days to work in an office, so this would be down to individual need to do so. Also need to be accessible to West Sussex to be able to do direct work with customers. It is important that a temp worker is confident with working in physical isolation but that they progress work and engage with support and supervision appropriately and that they are not adverse to going into an office.** Keen to find someone who has experience of working in transition- would consider others if CV was right and disability experience was strong
