This role includes:
Supporting decision making and assessing risks at the front door. Implementing risk management plans where necessary.
Providing professional support via mentoring and supervision to staff within the team ensuring professional standards and best practice are always upheld.
Working alongside and when required deputising for the team manager to prompt an environment of a supportive team enabling them and yourself to work with individuals to promote and support independence choice and control with wellbeing at the heart of everything we do.
Support front line practitioners in conducting individual complex assessments and safeguarding enquiries using the appropriate tools and techniques in order to establish clear, relevant, proportionate, and appropriate options for the individual to meet their identified outcomes.
Working in partnership with adults, their families, carers and advocates, colleagues, external organisations to provide appropriate, professional social work support for vulnerable older people, adults with physical and sensory impairment or learning disability and carers
Conducting Mental Capacity Act assessments in line with statutory guidance and ensuring adults are provided with the opportunity to demonstrate capacity wherever possible.
Ensuring that the responsibility of safeguarding adults at risk is a fundamental element of this job and that the principles are embedded in all procedures, practices, professional advice, decisions and support.

This 36 hour a week role offers hybrid working with a mixture of office and home working although staff are welcome to be office based fulltime if they wish and can access free parking where available .
