Social Worker - Mental Health Social Work Discharge Team

Job Description :
In this challenging, but rewarding role, you will be required to act as an
allocated social worker responsible for delivering a statutory social work service
to those people who access or need specialist mental health services and will
support their discharge from hospital. Your role will be to work in partnership
with the newly formed mental health hospital discharge hub including completion
of all statutory work such as Care Act assessments, s117 aftercare planning
meetings, safeguarding, Mental Capacity Assessments, Best Interest Meetings as
well as any other social care related tasks and functions. This will be a fastpaced role and you will be required to be proactive and able to work under
pressure to ensure all aspects of the individual’s needs are considered and
incorporated into the discharge process.
There will be some requirement for you to work flexibly in order to support
customers, carers and families, sometimes out of normal working hours.
You’ll need to be able to travel independently throughout the county, including
to areas that may not be easily accessible by public transport, so a driving
licence is essential. There is a regular and intrinsic requirement to communicate
in English with members of the public. You must be double vaccinated against

to prioritise needs and formulate plans for intervention and service provision.

colleagues and a variety of external agencies in order to deliver the most
effective service.

Qualifications and/or experience:
• Degree in relevant professional or equivalent qualification e.g. Certificate of
Qualification in Social Work or Diploma in Social Work.
• Registration with the relevant professional registration body and
responsibility for maintaining registration with that body.
• Approved Mental Health Practitioner (it is a requirement to have AMHP
qualification or to be working towards the qualification to be complete within
2 years of appointment and to maintain level of knowledge).
• Full, valid driving licence.
Job Description & Person Specification

• Theoretical, practical and procedural knowledge of social work. Good, up-todate working knowledge of legislation relevant to the specialist area of the
post e.g. Mental Health Act, Mental Capacity Act.
• Good understanding of the necessity of working with partnership agencies
including health, housing, learning, statutory, voluntary and independent
• Evidence of good assessment skills, including the ability to ensure regular
reviews and risk management with a variety of individuals, carers or families.
• Evidence of effectively prioritising work and meeting deadlines.
• Experience of working in partnership with those who access or need the
service and maintaining a focus on the needs of the individual whilst handling
conflict and disagreement.
• Experience of forming effective working relationships within a multi-agency/
multi-disciplinary setting.
• Evidence of working with those who access or need the service to identify
issues and identify specific action by means of creative and pragmatic
• Experience of managing levels of risk within a caseload.
We require 3 SWs to join this new team and will pay up to £32 per hour Ltd. The candidates would benefit from having experience in a hospital/mental health setting.
They will be working in the community, wards, care homes etc as well as in the offices and some homeworking.
They will be required to be doubly vaccinated and be physically present in West Sussex, long distance remote working is not feasible for this role. Please do not submit anyone who wishes to work from home.
