• Undertake regular looked after children statutory visits, assessments and reviewing looked after children Care Plans.
• Prepare reports and attend court in accordance with looked after children legislation and as directed by the team manager/line manager.
• To support children living in kinship placements, foster placements, prospective adopters and older children living in children's homes and semi independent units.
• To safeguard and promote the welfare of children, young people, and vulnerable adults.
• Nurture and develop meaningful relations with looked after children; understand and interpret their wishes, and champion and support them to reach their full potential.
• Attending panels and developing pathway plans for post-16 looked after children.
• Work collaboratively with professionals from both within the Children’s Services Division and other multi-disciplinary services to ensure that plans are carried out effectively in the best interest of children and families.
• Maintain accurate and full records of contact and interventions with children and their families in line with the department’s policies and procedures.
• Undertake child protection investigations and to provide assessments and recommendations that facilitate decision-making by appropriate line managers.
