Discipline
1. To carry out Needs- Led assessments of service users and their carers in accordance with the council’s Self-directed assessment.
2. To formulate appropriate care plans and negotiate care packages with service users and service providers. This will involve formulating multi-service care packages, and reviewing on a regular basis.
3. To carry out all duties in accordance with the council’s policy and procedures
4. To carry out needs led assessments of service users and their careers and to manage on a time limited or long term basis and meet performance targets.
5. To complete financial assessment of service users, and support them to manage their financial affairs.
6. To apply fair access criteria to determine eligibility for services.
7. To identify appropriate services to meet appropriate need, and negotiate contracts with providers.
8. To provide care plans to service users in receipt of commissioned services .
9. To regularly monitor and review carry commissioned services in accordance with the Community Care Act 1990.
10. To adhere to departmental targets. To comply with Borough of Brent policy on Customer journey processes.
11. To adhere to individual performance targets as set out in formal supervised/appraised meetings to ensure continuous excellent service delivery
12. To co-ordinate person centred support plans ensuring good practice guidelines are implemented for better outcomes and increased quality of life
13. To be familiar with Brent’s policies and procedures and appropriate legislation procedures and resources.
14. To maintain case records and to apply Borough of Brent case recording policy.
15. To contribute to the development of the team, by participating in planning and implementation of policies and good practice guidance.
16. To facilitate and foster effective relationship between Local Authority, NHS, private and sector partners and service providers. Co-operate and participate in the rota duty system at all times with other members of the service unit and to respond to Emergency situations when required.
17. To promote and ensure maximum service user/carer’s involvement and participation at all times.
18. To ensure that care purchases are set up to reflect customers need and are best value for money.
19. To maintain such reports and records as may be required, including car mileage expenses, hours worked, daily diary etc in accordance with Brent’s policy and procedures.
20. To participate in professional supervision with the Line Manager.
21. To undertake other duties, commensurable with the grade of the post as may be allocated by the post holder’s line manager
22. Carry our duties with due regard to the Council’s customer care, Equal opportunities information governance, Data protection and Health and Safety policies and procedures
23. Undertake any other duties commensurate with the general level of responsibility of this post
