Social Worker (Adults)-Vulnerability Assessment Team

The role of the Social Worker is to be able to work as an autonomous professional, demonstrating expert and effective practice in complex situations. This role will be supporting the Reviewing Cohort of the Vulnerability Assessment Team for adults across the East Riding of Yorkshire, we require experienced Social Workers in Adult Social Care to undertake dynamic Care Act care and support review and assessments, updating support plans and risk assessments focusing on a strength’s based approach to support and enable people and carers to actively stay well and independent, build and maintain family, social networks and use of technological devices to maximise independence.  The Social Worker should have working knowledge and experience of completing Care and Support Assessments, reviewing care packages, Mental Capacity assessments and use of the Best Interest Process.   A working knowledge of Safeguarding Adults at risk is essential.  Minimum of 2 years’ experience working as a qualified social worker in adult social care is essential.
