Social Work skill set required for successful candidate:
- Knowledge of key legislation relevant to professional social care work
- Knowledge of the functions of a health and social care department and related agencies,
- Knowledge of methods to plan and review services for the client group with focus on Reablement and strength-based approaches
- Knowledge of social diversity and equal opportunities and the ability to demonstrate this throughout all aspects of practice
- Should have minimum 4 years post qualification social work experience
- Experience of managing a complex case load
- Strong influencing and negotiating skills with clients and other health professionals
- Experience of producing reports for a variety of audiences
- Good IT skills and willingness to learn new systems
- Ability to organise and prioritise work and manage time effectively
- Able to work independently and within a team.
- Customer focused
***Ability to work face-to-face in the borough of Southwark and carry out home visits at least 3 days per week*** This is not a remote working-from home role**
Social Work role in an integrated Health and Social Care Team that covers short-term rehabilitation and Reablement for service users recently discharged from hospitals, rehab units and onto community therapy services specialising in stroke and neurological rehabilitation.
Intermediate Care Southwark is recruiting a qualified social worker (role not suitable for newly qualified social workers) to support the team with
- completing Care Act assessments
- reviews of long-term care provision
- mental capacity assessments relating to care support , change in accommodation and finances
Experience and skills preferred in areas of rehabilitation settings, dementia, neuro, stroke and Parkinson services, hospital settings and familiarity working in multidisciplinary teams and with older people and adults with physical disabilities.
- Background in CCG funding / Continuing Healthcare process desirable
NOTE: Rate can be increased to max. 32£/hour for suitable candidate with relevant experience
