We need an experienced Social Worker passionate about working with people to promote independence. We are looking for applicants with specific experience in Learning Disability and Mental Health work, and experience with complex needs.

We are looking for an enthusiastic social worker to work with adults with a Learning Disability, alongside their carers, family members and our Health colleagues from the Community Leaning Disability Team.
Applicants will have good communication and relationship building skills and fully recognise the importance of ensuring that the voice of those we are supporting is heard.
Candidates will need to use a strengths-based, person centred approach to promote independence and positive wellbeing, enabling the client to achieve their goals and aspirations.
They will be undertaking Care Act Assessments, Care Act reviews and creative support planning while working in partnership alongside other services from the third sector and community.
Workers will be required to take part in the Out of Hours (OOH) stand-by duty rota as and when requested by management on an occasional basis.
