Social Worker - Adults -Discharge

The Reablement Team works closely with the Admission Avoidance Discharge Service (AADS), a health team with whom they are currently working on moving to more integrated ways of working. Therapists in Reablement and AADS work with local residents for up to 6 weeks to progress independence within activities of daily living. To support this, reablement has its own in house reablement officers that provide day to day to support under the direction of an occupational/physio therapist, to encourage and support residents to carry out tasks themselves rather than carrying out tasks on their behalf. Some of our residents who work with Reablement/AADS have long term support needs and/or need safeguarding. We are therefore looking for strength focused Social Workers to complete Care Act Assessments, support planning and safeguarding work for service users who have ongoing social care needs after working with the AADS/Reablement team. This role presents an exciting opportunity to work within an integrated team with strong links to our health partners. The role is based within the Reablement Team within Adult Social Care and the successful candidate would be expected to work closely with Reablement and AADS staff. SW would also be expected to cover duty every third/fourth week, which includes responding to safeguarding concerns, urgent requests from AADS therapists to prevent hospital admission and any cases on the waiting list where there is an urgent need. The expectation is two days in the office (currently Mulberry Place in Poplar, moving to New Town Hall in Whitechapel on 20/02/2022) and three days working from home. Face to face visits to service users are completed as per practice guidance.
