We wish to attract a Social Worker who has experience in working with this client group and the associated strength based and outcome focussed processes and approaches and who is keen to be actively involved with the transformation process and the service redesign and new ways of working.
The focus on this role would largely on outcome focussed reviews and support planning with a view to use the outcomes where possible to stimulate and develop community resources, however you will also be expected to support and work closely with other professionals in the Transformation Team who will focus on achieving more independent outcomes for those in residential care and supported living settings as well as in transition and identifying common themes so the work in the various workstreams can be aligned.
You would be expected to work effectively and build positive relationships with all stake holders such as client’s carers, community connectors, care providers, direct care, and health. You will need to be committed to working in a person centred and strengths-based way with clients and their families to maintain and maximise their independence, whilst observing the tight timescales under the Care Act and current guidance in an outcome focussed way as part of Best Lives Derbyshire.
The role will involve undertaking research, identifying gaps in provision, and developing services in the community as well as liaising with personalisation and prevention teams and other ASCH disciplines to help develop these new ways of working
