|The sucessful candidate will be working with the Community Response Hub supporting individuals to transfer from an intermedicate care service (Reablement/Pre-assessment) onto a long term support plan. The candiate will be working within a busy team and be expected to work at pace while maintiainng a high standard of practice. The candiate will be supported informally daily with monthly supervision. Ideally, the candidate would have worked for Swindon Borough Council previsouly and/or be familiar with Eclipse. The candidate will be expected to complete face to face visits so should be able to commute to Swindon at short notice. We are looking to recruit 3 social workers for 3 months, one of these positions will be extended to 6 months if required.
