The Adult Social Care Team forms part of Southampton City Council’s Adults Housing and
Communities Directorate.
The service is responsible for triaging, signposting and delivering a range of support to adults who
are deemed to have a range of care and support needs, their families and carers-both informal and
formal. The broad areas of focus are:-
  • Strong partnership working with early help services to prevent vulnerable adults from needing more specialist help, building resilience in families and coordinating support
  • To lead coordinated offers of support to adults who are deemed to have care and support needs and are unable to protect themselves or others
  • Strong leadership and partnership with our other organisations to improve the outcomes for adults with care and support needs
  • Assessing adults with care and support needs or a disability and applying Care Act eligibility using a strengths based approach
  • Monitoring services, that will look for continual improvement that will provide care and support for our most vulnerable clients
  • Ensuring that services are compliant with statutory guidance and policy and that we are continually improving through work force development, performance monitoring, independent review and case audit
