MUST have a social worker qualification
MUST have experience in working within mental health
MUST have experience of working within Adult Social Care
MUST have experience of working within the Care Act, Mental Capacity Act, Mental Health Act, Safeguarding
IDEALLY would like candidates to have had experience in working in Older Adult Mental Health
MUST be a car driver
We are seeing patients Face to Face so MUST be able to travel and be prepared to Wokingham Borough area

Successful candidate will be responsible for monitoring the mental health of older people with both organic and functional mental health diagnosis. Be responsible for organising hospital discharges and assessing for care needs and arranging care as required. They will be responsible for their own case load and be expected to take part in the duty rota triaging referrals, taking calls as required and completing urgent assessments. This will be office based. A degree of home working is possible. Regular 1-1 supervision will be provided and weekly clinical supervision is also provided. We have a staff room and free onsite car parking.

The Wokingham Community Older Adults Mental Health Team is looking for an enthusiastic, caring and highly motivated Social Worker/Occupational Therapist (Mental Health Practitioner) to join our friendly and innovative Community Team
Your base would be Wokingham Community Hospital where we have access to free parking facilities, staff room and a friendly and welcoming team. There is also the opportunity to work remotely and a balance of the two would be encouraged.
The Wokingham Older Adult Mental Health Service is a multi-disciplinary team with specialist skills in caring for older people with mental health needs in the community. The post holder will take responsibility for the assessment and implementation of intensive care and treatment in the homes of people with functional mental health problems, as well as people with dementia.
