Social Worker -Adult’s - Assessment Team

36 hours per week, M-Th (9am -5.15pm ) & F (9am-5pm), Remote working with home visits, initial 3 months with extensions thereafter
The Team is now Community Active Recovery Team.
We are the initial assessment team, an experienced social worker would join us to support with community assessments, promote independence and enable service users. Transferrable skills to support discharge to assess during winter pressures may be required.
In this role, you will carry out assessments of social care needs and provision of support for vulnerable adults to ensure they are safe and have good outcomes to promote independence.
Duties of this Social Worker role include:
Manage our more complex cases both during their Reablement Programme and in transition to long term care
Manage safeguarding concerns and enquiries (with the support of Assistant Team Manager) and Mental Capacity Assessments
Required to be able to do MCA's, CHC and Safeguarding Assessments.
Complete assessments focussed on ensuring a smooth transition from hospital to home and that the individual is supported to remain at home and avoid re-admission.
Understanding of the Care Act and eligibility.
Able to carry out discharge to assess and Home First functional assessments
Assess eligibility for social care services and reablement to maximise independence and reduce need for ongoing care services.
Liaise with relevant agencies to oversee the case management, reviews, and service provision for up to 6 weeks.
Requirements of the Social Worker:
Degree or equivalent in social work
Current Social Work England registration
Significant senior level frontline experience within Adults
