Social Worker Adults-Safeguarding and Discharge

Must have experience of working in adult social care and confidence to manage complex cases.
*Positions available in Discharge to Assess team (D2Q), community team and safeguarding team.
*Posts are community based and will involve some home visits and candidates will be required to make visits in the local Portsmouth area.
Work patterns will be discussed at interview.

  • Assessment of adults with care and support needs and their carer's, identifying creative and personalised ways to provide support.  
Working with a range of individuals, often with complex and complicated needs:


 -Triage and prioritising of cases and telephone enquiries.


 -Participation and commitment to all aspects of adult safeguarding and  


   completion of S42 enquires when required.


 -Continuing Care assessments.


 -Care Act assessments.


 -Mental Capacity Assessments.
