As a Senior Social Work Practitioner in ACP2 you would be working alongside other Social Work colleagues within a multidisciplinary team including Occupational Therapists and Assessment Officers (AO). The ACP2 team is the access point to Adult Social Care in West Sussex. The teams’ key role is to undertake initial telephone Care Act Assessment. The ACP2 team begin, and in some circumstances complete, a statutory needs assessment with customers who are approaching WSCC for advice, guidance and support. Where ongoing face to face assessment is required customers are passed to locality OT or SW teams. Senior Social Work Practitioners in the team are responsible for managing the triage of referrals including SCARF and SECAMB alerts. The role also requires allocation of casework to colleagues, undertaking Care Act assessment of a complex nature, providing support and supervision to Assessment Officers and supporting the team with expert Social Work knowledge and skill. The role includes being responsible for leading on a specialist area of service delivery at the front door of Adult Social Care, ensuring that key developments are communicated to colleagues and that business change is successfully implemented. We are looking for a candidate who will be able to support and deputise for the Team Manager in terms of team performance, incoming demand management and ACP2 developmental planning. The postholder will work from home, but is required to attend induction in Horsham. £32 - £35 per hour Ltd, dependant on experience
