Intermediate Care Southwark (ICS) is recruiting a locum Senior Practitioner (SP) to work alongside 3 other SPs in the integrated service. Note: Post includes managing a small case load of complex cases alongside sharing Duty SP cover, authorisations of written work from other practitioners and supervisory role to a mixture of CSO/AP/SW staff.
Post holder would be accountable for assessment of complex social care needs and provision of support for vulnerable adults to ensure they are safe and have good outcomes to promote independence.
Accountable for working within the statutory legal framework, policy and guidance and determining when the threshold for statutory intervention has been reached in line with legislative and organisational standards within the Care Act 2014 (e.g. MCA/DoLS/CHC assessments etc).
Responsible for managing their own small complex caseload, maintaining accurate and timely data, reports and funding Panel presentations as required.
Accountable for conducting assessments and plan responses to safeguard vulnerable adults, including initial risk management and safety plan as required.
Person needs to be able to builds strong effective working relationships across the Council, with partners and members of voluntary agencies (e.g. AgeUk) as well as working in an MDM with Health colleagues in the integrated service.
Suitable candidate would bring a high level of expertise in particular areas of social work practice and leads on those areas, contributing to the development of the service via projects / work streams with other colleagues.
AMHP or BIA background would be beneficial but not essential.
