A dynamic experienced support worker is required to work with young people and their families in schools and in the community undertaking EHAT's to assess needs
Identifying a wrap around support plan in order to improve outcomes for young people and their parents/carers working on a one to one basis with children and young people and support and advise parents to improve parental capacity and bring about change
To provide direct support to children, young people and their families facing a range of difficulties. This role will provide additional support at an early stage to help divert families from requiring higher threshold services. The service will seek to achieve the following outcomes:
