1. To work with a caseload of young people aged between 17.6 – 21 years old (in some cases 25years old) to act as a Corporate Parent, providing advice, assistance and support.
2. To co-produce pathway plans with young people (assess, plan and review support) to enable young people to transition from leaving care into adulthood/independent living.
3. Act as a thematic lead for a specific area of work within the team and contribute your specific skills & knowledge
4. Effectively manage competing priorities and workload including meeting required practice standards and statutory responsibilities.
5. Practice in a multi-disciplinary environment and work in a supportive and collaborative manner with young people, colleagues and partner agencies to deliver cohesive services to young people.
6. Attend regular supervision, team meeting, team development days and contribute to the development practice guidelines, policies and procedures as necessary
7. Maintain accurate and up-to-date records using CASS system within practice standards. Ensure accuracy and timeliness of recording/data in order to ensure the service is meeting key performance indicators and targets.
8. Represent the service in a manner consistent with the highest professional standards and reflective of the service and its commitment to equality of opportunity
9. Remain aware of developments locally and nationally in terms of policy and practice and to demonstrate an ongoing commitment to continuous improvement and working to the highest quality standard.
