The role will play a key role in ensuring that residents receive the care and support that they required following a social care assessment in an effective and timely manner, ensuring the speed of placement is minimised and value for money is maximised
The team works remotely and whilst linked to one of the localities (East, Mid or West) will work flexibly to meet changing demand, both type of care and geography
This role is based within Adult Social Care as part of the Locality Commissioning Service to arrange packages of care/placements for adults with eligible social care needs. This includes:
• Home Care
• Care Homes
• Assistive Technology
• Transport arrangements
• Day Care
The role holder will use a range of systems and tools including Mosaic, dashboards, GIS mapping tools and the Dynamic Purchasing System to arrange packages and placements in line with Council Standard Operating Procedures.
The role holder will be required to liaise with providers and colleagues across the health and social care system to coordinate appropriate care provision. this will include seeking clarification from Referring Clinicians and Providers where necessary in respect of individual packages of care.
The role holder will work within challenging deadlines to deal with often urgent care needs and, due to the pressured needs of the Adult Social Care and Health systems, the Service operates over a 7-day working week on a rota basis
