The Southwark Telecare service is looking for an assessment officer to join our established Telecare Team on a temporary . You will bring your existing knowledge and expertise to provide an enhanced Telecare service to the wider Community. Technology Enabled Care is a growing and exciting sector within Social Care and offers opportunities for developing your skills and knowledge.
As an experienced assessor, you will complete Telecare assessments in the community and identify creative solutions including the provision of Telecare and stand alone assistive technologies to enable services users to remain in their own homes according to their wishes; and to promote independence, choice and well being and provide support to carers as appropriate.
Your strong organisational, problem solving, IT and report writing skills will be essential in assessing service users needs and producing comprehensive reports. You will also participate in the Telecare Duty process and will need to be skilled at prioritising and screening new referrals.

To work with the person and other professionals as necessary in order to facilitate the stated
functions. This includes the undertaking of duty activities and proportionate assessments,
safeguarding initial contacts and investigation, the onward referral to community teams for support
provision as well as signposting and the provision of information and advice. Responsible to the
Team Manager and Senior Practitioners. To ensure that all related administrative functions are
undertaken.
