Assistant Practitioner-needs of elderly people and people with disability.

Location Southwark
Salary £18.00 - £20.50 per hour
To work with the person and other professionals as necessary in order to facilitate the stated functions of the Reablement Service. This includes the undertaking of goal setting visits, proportionate assessments, onward referral to community teams for support provision as well as signposting and the provision of information and advice. Responsible to the Team Manager and Senior Practitioners.
Provision of a short term intensive assessment period, assessing the person’s ability to maximise their independence and to provide a robust assessment that measures the amount of on-going support that is needed. To ensure that all related administrative functions are undertaken.
Knowledge, including educational qualifications:

Knowledge of literacy and numeracy skills, to write factual reports to meet the needs of Agencies/ Departments, to maintain appropriate records including financial assessments and to assist people with their finances. To produce high quality assessments.
An appreciation and understanding of current issues in social work practice.
Ability to use IT systems to input and access data, use word processing systems and Email.
Understanding of and commitment to the Councils Equality and Diversity policy.

Experience of:
Assessing the needs of elderly people and people with disability.
Working with multiple agencies to facilitate holistic assessments. To collaboratively support people in achieving their goals, outcomes and maximisation of their independence.
Identifying safeguarding and quality of care issues. To ensure that clients’ welfare is protected and that the quality and standard of support provided is optimised.
